Have been listening to the podcast for ages, love it! I work for a 2500 person company that doesn't have a proper way for directs to evaluate their managers. Being new to the company, and management, I would find this sort of feedback very valuable. I've talked with some peers and they agree. We have the go-ahead from our director to give surveys to our old teams (we just reorg'd due to growth). I wanted to get some feedback from you on what might be some good questions to include on the form.

Three simple questions
For formal feedback, we use an externally moderated 360 tool, which I find quite useful. Unfortunately, I don't have soft copy and I can't tell you which one.
For informal feedback, we've gotten pretty good results asking three questions:
1) What three behaviors should I do more of?
2) What three behaviors should I do less of?
3) What three behaviors should I keep doing?
The results require a bit of interpretation, but this has worked fairly well. My boss asks my directs for this information (his skips), collates it, and uses it as part of my performance feedback.
